PASSPORT APPLICATION INFORMATION/SERVICES

Passport applications can be downloaded from the U.S. Department of State’s website at http://travel.state.gov/passport. There, you will find information on the identity documents and photos required, the fees charged, and answers to travel-related questions. Passport applications are also available at our office.

Do not sign the application, as this must be done in person. All applicants must appear in person, regardless of age. If you are obtaining a passport for a minor, he or she must be present along with both legal/custodial parents. If both parents cannot be present, you will need to provide additional written authorization (please call ahead for further details).

Passport Acceptance Hours:

By appointment only:

Monday – Friday  9:00 am to 4:00 pm

Payment: We accept money orders or personal checks.  NOTE: two or more checks are required for each application – one made out to the Town of Sudbury and one made out to U.S. Department of State for EACH application.  The Town of Sudbury receives a processing fee of $25 per application.

Onsite Photos:  Passport photo service cost $7 per set.  May pay cash or check payable to Town of Sudbury.

Application fees to the Federal government vary depending on services requested and age of application.  Please call our office for further details or go to the travel site for more information http://travel.state.gov/passport.

Checklist for coming to our office:

  • Call ahead for appointment to make sure someone is available to process your application
  • Application completely filled out but NOT signed
  • Checkbook (you will need separate checks for each application and one for the Town)
  • Proof of Citizenship (original, certified.  Copies not accepted)
  • Proof of name change (if applicable.  Original document, copies not accepted)
  • Government issued photo ID (i.e. Driver’s license)
  • All applicants must appear in person.  If applicant is a minor, generally both parents must accompany child.  See Federal website for all rules and regulations pertaining to passport processing for minors.
